In order for us to design a sound financial strategy, as we move through the process, we will need the following documents (see Important Documents below). Your documents will be held in a confidential manner while they are being reviewed. They will be returned to you as quickly as possible. If you prefer, feel free to provide copies of your legal documents and financial papers for our meeting.

The privacy and confidentiality of your personal information is very important to us at Spak Financial Services. We adhere to all privacy and confidentiality requirements for all entities whose products or services we offer.

1. Income Tax Return(s) for last year

2. Paycheck Stub(s) for you and your significant other showing deductions from gross income

3. Most recent statement for each investment you own

4. Copies of your most recent retirement plan statements

5. All Insurance Policies (please include declarations of coverage)

  • c. Life Insurance Policies (for all members of your family)
